Make a Referral

Professionals & Commissioners We welcome referrals from social workers, local authorities, commissioners and other professionals looking for safe, high-quality supported living for young people aged 18–25. Our referral process is simple and responsive: * Send us the young person’s referral and support information. * We review their needs, risks and desired outcomes. * We complete an initial suitability and compatibility assessment. * We discuss funding, placement requirements and any specialist support needed. * Where appropriate, we arrange a visit or introductory meeting. * We agreed a personalised support plan before move-in. * We work closely with professionals throughout the placement. * We regularly review progress, outcomes and changing support needs.
